    Travel Immunisations

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements.  This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

To help us offer the appropriate advice, please fill out the Travel Immunisation Form, which can be downloaded in the ‘Forms’ section of this website. Please then complete and print the form and hand in or email this through to the surgery.

It is important to make this initial appointment as early as possible  – at least 6 weeks before you travel – as a second appointment may be required to receive further vaccinations.  Some vaccines may have to be ordered as they are not kept in stock and some vaccines may incur a charge.

There is further information about countries and vaccinations required by following the links below:

Please note should you decide to email this form you will be sending information about yourself across the internet. Whilst every effort is made to keep this information secure, you should be aware that we cannot offer any guarantee of absolute privacy. If this matter concerns you then you should use another method to notify us of your information.
